- 上気道粘膜の急性カタル性炎症(急性カタル性上気道炎)の総称
- 急性鼻炎や咽頭炎を呈し、全身症状は軽微。
- 予後良好、2-5日で軽快し予後は良好。
病因
- 病原体への暴露：飛沫感染など
- 環境要因：個体条件(アレルギー、免疫不全、脱水、疲労、飲酒など)、物理化学的刺激(乾燥、寒冷)
疫学
- 多くの人が1年に1回以上罹患。冬期に多い。
- 小児において罹患回数が多い → 成人になると記憶T細胞、記憶B細胞が増加するため、なんとなくウイルスに対して抵抗性が付与される、はず。
病理
- ウイルスの場合、上気道粘膜に付着して、粘膜上皮を冒す。上気道線毛円柱上皮(呼吸上皮)に付着し、上皮細胞は変性・脱落・壊死する。上皮を失った部位から細菌が侵入しやすくなる。
病型
- 鼻かぜ：ライノウイルス、RSウイルス、コロナウイルス
- のど風邪：アデノウイルス、コクサッキーウイルス、パラインフルエンザウイルス
- 気管支かぜ：アデノウイルス、パラインフルエンザウイルス、マイコプラズマ、クラミジア
症状
- 鼻かぜ：鼻汁、鼻閉などの鼻症状が主体
- のどかぜ：咽頭痛、嗄声など咽頭症状が主体
- 気管支かぜ：咳、痰などの気管支症状が主体
共通の症状
- まず、鼻炎症状が緩徐に発現し、鼻咽頭不快感・乾燥感、くしゃみ、鼻閉、水様鼻汁を呈する。
病原体に特異的な症状
- ライノウイルス・コロナウイルス：2-3日の鼻炎症状で全身症状は軽度。
- アデノウイルス：咽頭炎・喉頭炎、ヘルパンギーナ、下気道感染、結膜炎
- エンテロウイルス：夏かぜ。腸管症状。
- RSウイルス・パラインフルエンザウイルス：成人では軽症だが、乳幼児でときに重症 → 細気管支炎
診断
- 臨床的診断
- 検査はほとんどの場合行わないが、インフルエンザ、溶連菌などを鑑別する場合には迅速診断キットを使う。 → つかえる迅速診断キットがあればのはなし
検査
- 血液検査：ウイルス感染の場合、WBCやCRPは動かないが、細菌感染があれば上昇する。
- ウイルス分離は一般臨床では行われない。 → 高コストだし、培養している内に治癒する。
- 血清抗体価測定ではペア血清で 4 倍以上の上昇で陽性とする。 → 高コストだし、ペア血清を取った時点で治癒する。
- 細菌感染を疑ったら、喀痰検査(培養は時間がかかるので検鏡でしょう？)、血液検査(血算)のちに、培養して薬物感受性検査を行う(けど時間がどのくらいかかるのやら)
治療
- 一般療法(安静、保温・保湿、栄養補給、脱水予防、入浴制限)
- 対症療法：解熱・鎮痛薬、非ステロイド性抗炎症薬、蛋白分解酵素製剤、含嗽薬、鎮咳去痰薬、抗ヒスタミン薬
- 細菌感染が疑われるときのみ、抗生物質を使用する。予防的に使うのはいかがな最中
予後
- 良好。数日-1週間で軽快
予防
- RSウイルス：適応が限られるが、ワクチンを使うことができる。
「急性上気道炎」
- ウイルス性、細菌性がありうるが、抗菌薬投与は細菌性のみに対して行う。溶連菌による咽頭炎に対してはリウマチ熱を行うために抗菌薬を投与する。PSAGNに対する予防、あるいは咽頭痛、発熱に対する緩和効果はほとんど無いらしい。
