「先天性胆道拡張症」
概念
- 先天性胆道拡張症とは先天的に肝外胆管が嚢状・紡錘状に拡張した疾患。(SSUR.612)
疫学
- 男女比=1:3
病態
- 乳幼児期に発症することが多いが、成人期まで無症状のことがある(SSUR.612)
- 10歳までに発症
- 膵胆管合流異常 pancreatico-biliary maljunctionが多くの場合認められる。ほぼ100%らしい。
- 高率に胆管癌を合併。
症候
- 三主徴：腫瘤触知、腹痛、黄疸
治療
- 以前は胆嚢の出口？に問題があったと考えられていたので、胆嚢から腸管に胆汁をバイパスする手術が行われていた。
胆管癌の予防
- 高率に胆管癌を合併するために、積極的に胆管切除(嚢腫切除)、胆嚢摘出、胆道再建術(SSUR.612)が必要。
- 手術は、トライツ靭帯より肛門側で空腸を切断し肝臓に吻合。先ほどの空腸と肝臓を吻合した空腸を端側吻合。
