概念

巨赤芽球性貧血の中の分類の一つ
悪性貧血は内因子の欠如に伴うコバラミンの障害吸収による貧血と定義される
実際には、内因子を産生する壁細胞の喪失を伴う自己免疫疾患である。
アジソン病患者は、悪性貧血の場合と同じような萎縮性の胃炎を示すが、内因子の分泌は続く

病因

ビタミンB12欠乏症が本態

疫学

10歳以下の子供と60歳代の老人

徴候

毛髪：(若年の場合)白髪
舌　：疼痛、発赤、乳頭萎縮
眼　：眼球結膜の黄癬
皮膚：黄染、蒼白
循環器：蒼白、頻脈
消化器：食欲不振、下痢
神経：しびれ感、腱反射の減弱、位置覚・振動覚の減弱

神経症状

亜急性連合性脊髄変性症 subacute combined degeneration of spinal cord
末梢神経ニューロパチー、錐体路徴候、極端な場合は対麻痺、視神経萎縮、認知症

検査

血算

macrocytosis with hypersegmented neutrophils

大球性性貧血、大球性高色素性貧血
好中球：過分節()。好中球数減少
血小板数減少

「表在反射」

英: superficial reflexes
関: 皮膚反射

皮膚あるいは粘膜受容器を刺激することにより生ずる反射性筋収縮であり、多シナプス反射である。
錐体路徴候の一つあり、錐体路が傷害されると表在反射が消失する

分類

粘膜刺激によるもの

皮膚刺激によるもの

睾挙筋反射：L1,2。大腿内面を軽くこすると同側の睾丸が挙上する。
手掌反射
肩甲反射
肩甲間反射
腹壁表在反射/腹皮反射
腹壁皮膚反射/腹壁反射：上部(T6-T9)、中部(T9-T11)、下部(T11-L1)で腹壁の外側より正中に向かってこすると、正中線が刺激側に移動する。
殿部反射
足底反射：(バビンスキー反射)L5-S2。
肛門表在反射

肛門反射：S4,5。肛門又はその周囲の痛覚刺激、あるいは肛門に指を挿入すると肛門八ツ役筋の収縮が見られる。

概念

乳幼児や錐体路障害を来した成人で見られ、足裏外側をなぞった際に起こる足指を反らしながら開く反射(足底刺激に対して母指が背屈する(PT.123))である。健常な人では母指が底屈する。乳幼児の場合、抑制系ニューロンの未発達により生じ、成人では上位運動ニューロンの障害により生じる。生後から見られおよそ1歳(8ヶ月～2歳)(G10M.339)で消失する。
