貧血、巨赤芽球性貧血

概念

• 巨赤芽球性貧血の中の分類の一つ
• 悪性貧血は内因子の欠如に伴うコバラミンの障害吸収による貧血と定義される
• 実際には、内因子を産生する壁細胞の喪失を伴う自己免疫疾患である。
• アジソン病患者は、悪性貧血の場合と同じような萎縮性の胃炎を示すが、内因子の分泌は続く

病因

• ビタミンB12欠乏症が本態

疫学

• 10歳以下の子供と60歳代の老人

徴候

• 毛髪：(若年の場合)白髪
• 舌　：疼痛、発赤、乳頭萎縮
• 眼　：眼球結膜の黄癬
• 皮膚：黄染、蒼白
• 循環器：蒼白、頻脈
• 消化器：食欲不振、下痢
• 神経：しびれ感、腱反射の減弱、位置覚・振動覚の減弱

神経症状

• 亜急性連合性脊髄変性症 subacute combined degeneration of spinal cord
• 末梢神経ニューロパチー、錐体路徴候、極端な場合は対麻痺、視神経萎縮、認知症

検査

• 血算

macrocytosis with hypersegmented neutrophils

• 大球性性貧血、大球性高色素性貧血
• 好中球：過分節()。好中球数減少
• 血小板数減少
