概念
適応
(1)
- (1)気管挿管が不可能な場合や(2)2週間以上の長期挿管が必要となる場合
- 例：上気道閉塞(口腔咽頭領域の腫脹や腫瘍など)、頭頚部手術症例(術後出血の可能性を有する症例、呼吸管理が必要となる症例)、人工呼吸管理が必要な症例(重症肺炎、筋神経疾患など)などがあげられる。絶対禁忌となる症例はないが、出血傾向や凝固異常がある症例ではできる限りその改善を待って行うべき。
- 1. 遷延性意識障害患者の気道確保
- 2. 長期人工呼吸
- 3. 頻回の気道の吸引・洗浄が必要な場合
- 4. 上気道狭窄・閉塞(外傷、炎症、腫瘍、異物など)
- 5. 誤飲、誤嚥の予防、
- 6. 心肺蘇生時
- 7. 頭頚部悪性腫瘍などの手術時
SOTO.593
1. 気道閉塞
- 目的：気道閉塞の防止
- 適応：喉頭狭窄、口腔・喉頭の炎症性主張、顔面・頭頚部外傷あるいは新生物による閉塞、両側舌下神経麻痺、両側反回神経麻痺(声帯正中位固定症)、甲状腺・気管周囲リンパ節腫脹による気管の圧迫性閉塞、上気道領域の手術後一時的処置、咽頭・喉頭手術の前処置
2. 下部気道における分泌物貯留の処置と予防
- 目的：頻回の気道吸引、誤嚥防止
- 適応：延髄球麻痺、重症筋無力症、各種の原因による意識障害、頭頚部外傷など。特に経口的気管挿管による管理がが1週間以上の長期に及ぶ場合(声門の保護)。
3. 呼吸不全
- 目的：死腔の減少(50%減少)、気流抵抗(声門による)の減少、人工呼吸器による加圧呼吸
- 適応：中枢性・末梢性神経機能不全による呼吸不全、慢性呼吸器疾患、重症胸部外傷
4. その他
- 気道異物の摘出(下気管支鏡)
- 経口挿管不能例
- 閉塞型睡眠時無呼吸症候群の永続的な治療

- 甲状腺の峡部を下方に圧排し、甲状腺の上方で気管切開を行う。
- 成人の気管切開では原則上気管切開である。
- 欠点：気道狭窄の原因となることがある。
- 甲状腺の峡部を切断した上で気管切開する。
- 利点：安全で確実な方法
- 欠点：甲状腺切断部の止血が必須
- 甲状腺を上に圧排し、甲状腺の下方で気管切開
- 欠点：肥満で短頚の患者では難しいことがある

手順
- 1. 可能なら気管挿管し、局所麻酔 or 全身麻酔する。(緊急時には無麻酔もありうる)
- 2. 皮膚横切開。(横切開でも縦切開でもokだが、前者の方が術後の瘢痕が残りにくい)
- 輪状軟骨の下縁約2-3cmで5cmの皮膚を横切開する(SOTO.595)。
- 輪状軟骨の下縁から約2-5cmの皮膚を縦切開する(SOTO.595)。
- 3. 軟部組織を鈍的剥離(剥離は縦方向に行う)
- 4. 舌骨下筋群を左右に分ける。
- 5. 必要があれば、甲状腺峡部を結紮し切離。
- 6.十分な止血
- 7. 気管切開
- 通常、2-3気管軟骨部で行う。2-4気管輪の高さ(SOTO.596)とする資料もある。
- 切開法：縦切開、十字切開、逆U字切開など
- 低圧カフ付きのプラスチックカニューレ、金属カニューレ
- 9. カニューレ口に当てたガーゼ片が呼気時に翻転することによりカニューレの適切な挿入を確認(SOTO.596)。
- 10. 皮膚縫合
- 皮下気腫防止のために皮膚縫合は密に行わないようにする(SOTO.596)
