概念
- 髄液の産生、循環、吸収の不均衡により頭蓋内に異常に髄液の貯留した状態。
分類
発症の起点となった時期
- 先天性水頭症：ex. 中脳水道狭窄症、ChiariII型奇形(脊髄披裂、脊髄髄膜瘤)、Dandy-Walker症候群、Toxoplasmaなどの胎内感染
- 症候性水頭症、後天性水頭症：
- 腫瘍((小児)松果体腫瘍、髄芽腫)
- 出血：クモ膜下出血
- 炎症：髄膜炎((緩徐進行)結核性髄膜炎 クリプトコッカス髄膜炎)
直接の原因
- 脳出血に伴う脳室内血腫
- 髄芽腫・上衣腫などの後頭蓋窩腫瘍(第四脳室腫瘍)
- 頭蓋咽頭腫・胚腫・視床下部神経膠腫などの第三脳室腫瘍・脳室炎などによる中脳水道閉塞
- 結節性硬化症、トキソプラズマ症、Dandy-Walker症候群(後頭蓋窩正中の嚢胞、小脳虫部の形成不全)
脳圧
機序
- 1. 髄液の過剰産生 →(稀)脈絡叢乳頭腫
- 2. 脳室、くも膜下腔の通過障害 →脳室周辺の腫瘍、血腫、先天奇形
- 3. 上矢状静脈洞への髄液の吸収障害 →くも膜下出血、髄膜炎
病因
- 先天性：高頻度(中脳水道狭窄症(非交通性)とChiari II奇形(非交通性・交通性)による水頭症)、Dandy-Walker症候群、Toxoplasmaの胎内感染症
- 後天性：腫瘍(膠芽腫などの第4脳室腫瘍・松果体部腫瘍)(非交通性)、出血(クモ膜下出血)(交通性)、髄膜炎(交通性)
症候
- 新生児・乳児：頭皮静脈の怒張、頭囲拡大、頭蓋縫合離解、落陽現象
- 小児・成人：頭痛、嘔吐、うっ血乳頭などの脳圧亢進症状
- 正常圧水頭症：認知症 dementia、歩行障害 gait disturbance、尿失禁 urinary incontinence → D U G

治療
- 第一の治療はシャント手術：脳室-腹腔シャント(V-Pシャント)、脳室-心房シャント(V-Aシャント)、腰部-腹腔(L-Pシャント)
- 悪性腫瘍が原因である場合は原疾患の治療も。
