asteroid bodies. H&E stain.
An asteroid body, is a microscopic finding seen within the giant cells of granulomas in diseases such as sarcoidosis and foreign body giant cell reactions.[1]
There is controversy about what they are composed of. Traditionally, they were thought to be cytoskeletal elements and to consist primarily of vimentin.[2] However, more recent research suggested that that was incorrect and that they may be composed of lipids arranged into bilayer membranes.[3]
They were also once thought to be related to centrioles,[4] an organelle involved in cell division in eukaryotes.

病型
病理
- 真皮および皮下組織に、結合組織に被包された類上皮細胞の特徴的な島嶼状結節性の肉芽腫がある。
- 皮膚では複数の類上皮細胞肉芽腫が主に真皮の上中層に生じる。ときに真皮深層、皮下組織、筋層に生じうる。
症候
- 無症状での検診発見例が大多数。微熱などの症状を出して来院することはまれ。
- 肺病変 :両側肺門リンパ節腫脹(bilateral hilar lymphadenopathy, BHL):胸部X線所見の割には無症状。
- 眼病変 :両眼性霧視(ぶどう膜炎、硝子体炎)
- 心病変 :心サルコイドーシス。心ブロック、不整脈、Adams-Stokes症候群
- 皮膚病変:2型ある。結節型、びまん浸潤型が多く、古い瘢痕(肘、膝)にサルコイド肉芽腫ができることがある。この肉芽腫を病理診断に使うと浸襲が少ない。
- 内分泌異常:高ACE活性、高カルシウム血症
- その他・・無痛性表在リンパ節腫大や心病変(まれだが本症の急死の原因)脳神経(致死的)
病変部位の頻度
- 肺(縦隔・肺門リンパ節,肺)95%以上
- 眼(ぶどう膜,網膜,硝子体)30-40%
- 皮膚5-10%
- 表在リンパ節腫脹のみられることがあり,時に心臓病変がみられる。

- 初発症状:眼(眼前霧視)50%、呼吸器症状15%、皮膚症状5-10%、両側耳下腺腫脹、リンパ節腫脹
検査
- 生検:病理的に非乾酪性類上皮細胞肉芽腫を証明する。経気管支的肺生検や前斜角筋生検が行われる。
- 画像検査
- 胸部単純X線写真:両側肺門部リンパ節腫脹(BHL)、びまん性陰影、結節影、空洞病変、網状影、浸潤影、小粒状影
- 分類(I~III型):(I型)BHL、(II型)BHL+肺野病変、(III型)肺野病変。I型が最多。
- 胸部CT:縦隔リンパ腺腫脹、血管・気管支周囲の肥厚・不整像、胸膜不整像
- ガリウムシンチグラム(67Ga):病変部に集積。
- テクネシウムシンチグラム(99mTc):病変部に欠損像
- タリウムシンチグラム(201Tl):灌流欠損像
- PET:高集積
- 気管支鏡検査:気管分岐部周囲や両主気管支に毛細血管の増生・拡張や腫大したリンパ節による圧排像を認めうる。気管支内にサルコイド結節(肉芽腫)を認めうる。
- 気管支肺胞洗浄
- 細胞数増加
- リンパ球増加
- CD4/CD8比:高値 (喫煙者でも上昇しうる) → 液性免疫優位。血清γグロブリン上昇はこのためか?
- 皮膚生検、皮下結節生検、表在リンパ節生検、前斜角筋リンパ節生検、肝生検、縦隔・肺門部リンパ節生検:非乾酪性類上皮細胞肉芽腫の証明
診断
診断基準
- 参考1
- 組織診断群(確実) :1-(2)のいずれかの臨床・検査所見があり,1-(3)が陽性。
- 臨床診断群(ほぼ確実):1-(2)①,②のいずれかの臨床所見があり,1-(2)③の(a)(ツベリクリン反応)又は(c)(血清ACE)を含む3 項目以上陽性。
- (1) 臨床症状:呼吸器症状(咳・息切れ),眼症状(霧視),皮膚症状(丘疹)など。

- 類上皮細胞からなる乾酪性壊死を伴わない肉芽腫病変生検部位(リンパ節,経気管支肺生検,気管支壁,皮膚,肝,筋肉,心筋,結膜など)。クベイム反応も参考になる
治療
- stage0やⅠ(BHL)では経過観察
- stageⅡ(BHL+肺陰影)、Ⅲ(肺陰影のみ)、Ⅳ(肺繊維化)では
- 1ステロイド(8割) 2免疫抑制剤メソトレキセート・アザチオプリン(難治性2割に)
予後
- 70%は2年以内に自然寛解。5-10%は難治性に進行。
- 死因の60%は心サルコイドーシスであり、心病変(刺激伝導障害)をきたす。
