バイタルサイン(vital signs)は、生命兆候という意味の医学・医療用語である。
日本の医療従事者の間では、(特に現場で)バイタルサインのことを「バイタル」と略して呼ぶことがある。

概要
「vital signs バイタルサイン」は医学・医療用語であり、「vital バイタル」は「生きている」、「sign サイン」は「兆候」という意味で、つまり人間が生きている状態であるということを示す兆候(=生命兆候)を意味する[1]。
英語では複数形で「vital signs」とされるように、兆候はいくつもある。人間が生きている場合には次のような兆候がある[1]。
- 心臓が拍動し[1]
- 血圧が一定値以上に保たれ[1]
- 息(呼吸)をし[1]
- 体温を維持し[1]
- 排尿・排便し[1]
- 意識状態に応じて反応し[1]
- 脳波が特定パターンを示す[1]
測定
- 4つのバイタルサイン
「バイタルサインの測定」と言う場合は[1]、通常、それらの兆候の中から血圧・脈拍数・呼吸速度・体温の測定を行っている[1]。 そしてそれらを数値で表す。
- 第5、第6
測定する時には基本的には上記の4つの兆候が選ばれることが一般的なわけであるが、加えて他の1~2の兆候も併用されることがあり、それらは「fifth vital sign 5つ目のバイタルサイン」や「sixth vital sign 6つ目のバイタルサイン」などと呼ばれている。
5つ目や6つ目としては、たとえば、瞳孔の反射を診ることも行われる。尿量を含める場合もある。救急医療ではバイタルサインとして意識レベル(意識スケール)も重要視している。最近ではパルスオキシメーターによる動脈血酸素飽和度(SpO2)の測定値も含めることがある。
- 測定に用いる機器
基本の4つは、体温計・血圧計・腕時計の3つがあれば測定することができる。(ただし、脈拍は基本的には手指でとれるが、あまりに弱い場合は聴診器が必要となる)。
病院内でバイタルサインをモニタリング(継続的に測定・記録)する場合には、英語で「medical monitor」や「anaesthetic machine」と呼ばれ、日本語では「バイタルサインモニタ」「生体情報モニタ」「ベッドサイドモニタ」「臨床モニタ」等と呼ばれる装置が使われている。
