概念
- 主に男性に見られる慢性かつ進行性の疾患であり、レム睡眠における筋弛緩の欠如と粗暴な行動の出現が特徴的である。
疫学
- 50-60歳以上の男性に多い。
病因
- 出典無し
症状
- 日中には問題はない。
- レム睡眠中に筋弛緩を欠いており、夢の内容をそのまま行動に移す結果、殴る、蹴る、跳ねる、ベッドから飛び出すなどの粗暴な行動が起き、自分自身あるいは家族を傷害することがある。
- 声をかけると覚醒し、目覚めたときの意識と見当識は正常で、異常行動を呈した場面を想起できることが多い。 ⇔ 譫妄、夜驚
診断基準
- 睡眠障害国際分類第2版(ICSD-2)
- A.
- 少なくとも次の1項目が存在する．
- けがをしてしまうか，あるいはけがをしてもおかしくないような睡眠時の荒々しい行動．
- 睡眠ポリグラフ検査中に記録されたREM睡眠時の異常行動．
- B.
- 筋抑制を欠いたREM睡眠(REM sleep without atonia)が存在する：持続的または間欠的な頤筋活動
の亢進や，頤筋または四肢の相動的な収縮(phasic EMG twitching)．
- C.
- REM 睡眠中に，てんかん性脳波活動がみられない(RBDと他のREM 睡眠関連性けいれんの併発とを，明瞭に区別
できる場合を除く)．
- D.
- 他の睡眠異常症では十分説明できない睡眠障害である(内科疾患や神経疾患，精神疾患，薬剤性や薬物乱用による疾患)
