概念
- 出産直後から進行性の呼吸不全を起こす代表的疾患。
- 胎児の未熟性(在胎34未満)による肺サーファクタントの産生障害により、肺胞の虚脱および硝子膜形成に基づく換気障害である。肺胞の拡張不全によりアシドーシスを来たし、肺血管の収縮により低酸素血症が悪化する。また、アシドーシス・肺血流減少、肺サーファクタント不足により肺胞上皮が障害をうけ、透過性の亢進により肺硝子膜が形成される。結果として呼吸不全となる。(SPE.99 NGY.503)
