分類
- 全子宮破裂：子宮壁全層が断裂して子宮腔と腹腔とが交通するもの。
- 不全子宮破裂：裂傷が子宮筋層のみにとどまり漿膜に及ばないものをいう。
原因

- 産科手術(鉗子分娩、過度の子宮内掻爬術)、陣痛促進剤の誤用、外傷
症候・身体所見

- 切迫子宮破裂：Bandl収縮輪の上昇、過強陣痛、激しい腹痛、不穏状態、胎児機能不全
- 完全子宮破裂：内出血と腹膜刺激症状によりショック症状。外診上、子宮と胎児を振れる
- 不全子宮破裂：広間膜内から広がる広範な血腫が形成される。
診断
- 参考1
完全子宮破裂は瘢痕子宮や過強靭痛などで診断が容易であるが、自然子宮破裂は診断が困難で、胎児分娩後に診断されることが多い。子宮収縮良好で外出血が少ないにもかかわらずショック症状が見られたら不全子宮破裂が疑われる。このため、超音波検査やCR,MRIを施行する。
治療

- 切迫子宮破裂：帝王切開による急速遂娩
- 完全子宮破裂：ショックに対して補液・輸液を行い、迅速に開腹手術を行う。子宮全摘、あるいは破裂部の縫合を行う。
- 不全子宮破裂：血腫除去手術
