Nesiritide (Natrecor) is the recombinant form of the 32 amino acid human B-type natriuretic peptide, which is normally produced by the ventricular myocardium. Nesiritide works to facilitate cardiovascular fluid homeostasis through counterregulation of the renin-angiotensin-aldoesterone system, stimulating cyclic guanosine monophosphate, leading to smooth muscle cell relaxation.

Neseritide was believed initially to be beneficial for acute decompensated congestive heart failure. It received approval from the United States' Food and Drug Administration for this purpose in 2001 after initial non-approval. In July 2011 the results of the largest study so far for nesiritide was published in The New England Journal of Medicine; it showed that nesiritide was not associated with a change in mortality or re-hospitalizations.^[1]

Administration

Nesiritide is only administered intravenously, usually by bolus, followed by IV infusion. For most adults and the elderly, a normal dosage is 2 µg/kg followed by a continuous IV infusion of 0.01 µg/kg/min. This may be increased every three hours for a maximum of 0.03 µg/kg/min.

Side effects

Common side effects include:

Low blood pressure (11% of patients)
Headache
Nausea
Slow heart rate
Kidney failure^[2]

More rare side effects include:

Confusion
Paresthesia
Somnolence
Tremor

薬物療法

以下、左室収縮能不全に対する治療のみ採り上げる

左室収縮能不全の原因は非虚血性の拡張型心筋症と虚血性心筋症に大別できる。これらの疾患においては交感神経系とRAA系が賦活化され、進行性の左室拡大と収縮性の低下、すなわちリモデリングが生じ病態の悪化に繋がっていると考えられている。治療の焦点は神経内分泌系の抑制、心不全に伴う症状の緩和である。
(PHD.244)慢性心不全の治療のゴールは5つある：(1)病因の同定、(2)増悪因子の除去、(3)心不全症状の管理(前方障害、後方障害の軽減)、(4)神経ホルモン系の調節、(5)長期予後の改善

治療に用いられる薬物

参考(1))

配糖強心薬(ジギタリス)
利尿薬：静脈還流量の減少　→　前負荷の軽減　→　左室拡張終期圧低下　→　肺うっ血の改善　→　低血圧誘発がありうる
血管拡張薬(vasodilator)

venous dilator(ex.硝酸薬)：静脈のcapacitanceを増加させる。利尿薬と同じ薬理作用。
arteriolar dilator(ex.ヒドララジン)：後負荷を低下させSVを上昇させる。血圧は下がりそうだが、SVが上昇する結果、血圧は軽度低下あるいは保たれる(PHD.246)
"balanced vasodilator：「アンジオテンシン転換酵素阻害薬」心不全で起こる血管収縮、体液貯留(volume retension)、心室モデリングを防ぎ、回復(reverse)させうる？CHF患者の生命予後改善効果がある(PHD.245,246)。ACE阻害薬は循環血液中のブラジキニンを増加させる。ブラジキニンは心不全の患者において血管拡張という重要な働きをしている(PHD.247)。「アンジオテンシンII受容体拮抗薬」はACE阻害薬とほぼ同等であり、ACE阻害薬による副作用が問題となる場合に用いる。(PHD.247)。これらの薬剤は腎不全と高カリウム血症が存在する場合には適さない。「nesiritide」human recombinant B-type natriuretic peptide。静注薬。速効で強力な血管拡張、心拍出量増加、RAA系の抑制、利尿作用(PHD.247)

~~カルシウム拮抗薬~~：長期に用いると心不全を悪化させる危険があり推奨されない。

経口強心薬
ナトリウム利尿ペプチド：ANP製剤(カルペリチド)　→　静注薬しかないので急性心不全の時に用いる。

血管拡張薬：硝酸イソソルビド、ヒドララジン
利尿薬：フロセミド、アゾセミド、トラセミド、スピロノラクトン、エプレレノン
ジギタリス製剤：ジゴキシン
