概念
- 尿細管においてアルドステロンの作用が低下した病態。アルドステロンは接合尿細管や集合管でナトリウム再吸収、カリウム排泄、酸排泄を促進するので、低アルドステロン症ではこのアルドステロンの作用が減少することで本病態を引き起こす。
病因
- 1. 副腎皮質からのアルドステロンの分泌低下
- 2. 尿細管におけるアルドステロンに対する反応性低下

- 低ナトリウム血症、高カリウム血症
- 尿中ナトリウム高値、尿中カリウム低値
- 血中HCO3-低下
- 糖尿病性ケトアシドーシスに糖尿病性腎症による低アルドステロン症が合併している場合、アニオンギャップの低下以上に重炭酸イオン濃度が低下する。この差が低アルドステロン症による過剰の重炭酸イオンの消費となる。
