リスク因子
- 高齢、潰瘍の既往、糖質ステロイドの併用、高用量あるいは複数のNSAIDの内服、全身疾患の合併 (ガイドライン1)
病態
- 幽門部から前庭部に多発する比較的小さな潰瘍、前庭部の深掘れ潰瘍、不整形の巨大潰瘍などが特徴 (ガイドライン1)
治療と予防
- ガイドライン1
- 非ステロイド消炎鎮痛薬(NSAID)は可能ならば中止し通常の潰瘍治療を行う。NSAIDの中止が不可能ならばプロトンポンプ阻害薬(PPI)あるいはプロスタグランジン(PG)製剤により治療を行う。
NSAID継続下での再発の防止には、PPI、PG製剤あるいは高用量のH2受容体拮抗薬(H2RA)が有効である。
- COX-1は胃粘膜などの組織で構成的に発現している。NSAIDの作用によりCOX-1の作用が抑制されると胃潰瘍を来しうる。投与経路(経口、坐薬)を変更してもこの副作用の発現は変わらない、らしい。
