概念
- ネグレリア属のなかでヒトに感染性を示すアメーバ
- 髄膜脳炎原因アメーバ
- 大きさ20um
- ヒトの鼻腔粘膜から嗅神経を経由して中枢神経系に浸入し、致死的な原発性アメーバ性髄膜脳炎を引き起こす。
- 急性(数日-1週間)発症で、健康な若年者が湖水などで水泳後に発症することが多い。1965年の初例報告以来世界的には数百例の報告がある (参考.3)

「原発性アメーバ性髄膜脳炎」
概念
- Naegleria fowleriの中枢神経への感染により急性発症
病原体
疫学
症状
- 突然の頭痛、発熱、嘔気・嘔吐 → 急激増悪で昏睡・痙攣 他の化膿性髄膜炎と鑑別が困難。ここ一週間、淡水で泳いでいないか問診が重要かも。
診断＆診断
- 炎症性中枢神経疾患の鑑別診断としてあげること。
- 確定診断は病理組織学的診断(病理染色、免疫組織染色)、PCRなど。髄液からのアメーバー検出
治療
- 薬物療法：アムホテリシンB、ミコナゾールの経口投与、静注の同時・大量服薬
予後
- 致命率は高い
予防
- 水泳中に鼻から水を吸い込まないように注意(参考文献.1) ← 無理無理
- 怪しげな淡水で泳がないこと！！
