関

頚椎後縦靭帯骨化症、後縦靭帯、難病

概念

• 椎体及び椎間板の後面にあり脊柱管の前壁をなす後縦靭帯が肥厚・骨化し、脊髄を緩徐に圧迫して脊髄症状を引き起こす疾患。
• 変性は頚椎に多いが胸腰椎のこともある。
• 1960年に月本により報告された。
• 難病であり、特定疾患治療研究事業の対象疾患である。
• びまん性特発性骨増殖症の部分症であることがある。

病因

• 遺伝によるものが指摘されている。

疫学

• 東南アジアに多く、50歳以上の男性に多い。男性での発生頻度は4%、女性では2%。
• 頚椎部の発生は男子に、胸椎部の発生は女子に多い。
• 日本人3%、中国人0.2-1.8%、台湾人3%、アメリカ人0.12%、ドイツ人0.1%。 (SOR.447)
• 糖尿病の合併例が多い
• 患者の両親に26%、兄弟に28%と同一家族に多発

• 骨化占有率が50%を越えると60%の症例に脊髄症が発生
• 骨化占有率が60%を越えるとほぼ全症例に脊髄症が発生

脊柱管の中の大部分を占める骨化像が認められる。単純X線側面像はわかりにくいが、脊柱の後方で濃度の高い部分がC3?-C5?に認められる。

• 頚部MRI

治療

SOR.449

重症例には手術療法をするが、軽症例に対して予防的に手術すべきかという基準はない。

• 保存療法：頚椎カラー装着により頚部の安静を保つ。
• 手術療法：除圧術により脊髄の圧迫を解除する。手指の痺れ・巧緻運動障害、下肢体幹の感覚障害、痙性歩行、膀胱直腸障害などの重症頚髄症が見られた場合には手術適応。
