By applying the evidence-based practice (EBP) process to diagnosis, screening, and referral, physical therapists can identify diagnostic and screening strategies that positively influence clinical decisions. Critically appraised topics (CATs), a standard one-page summary of the literature appraisal and clinical relevance in response to a specific clinical question, are a valuable tool within the EBP process. The creation of CATs makes educational processes cumulative instead of duplicative, allows the individual clinician to assimilate and consolidate knowledge after a search effort, and may improve search and appraisal skills. The purpose of this clinical commentary is as follows: (1) describe the clinical reasoning process of three orthopedic physical therapists which lead to the development of specific clinical questions related to screening for non-musculoskeletal pathology; (2) describe the search and triage strategy which led each physical therapist to the current best evidence needed to rule out non-musculoskeletal pathology in their patient; and (3) discuss advantages and disadvantages of CATs, implementation of this process, and tailoring search strategies to find diagnostic and screening strategies.
