経皮的心肺補助 PCPS

概念

• 先端にバルーンのついたカテーテルを大腿動脈から刺入し、先端が下行大動脈左鎖骨下動脈分岐部直下にとどまるように留置。
• バルーンにはヘリウムもしくは二酸化炭素が注入される。

メリット

• 1. 拡張期動脈圧が上昇　：バルーンが拡張　→　駆出された血液を停滞させる　→　冠動脈血流量の増加　→　冠状動脈の血流は拡張期圧に依存
• 2. 収縮期の駆出抵抗減弱：バルーンが収縮　→　バルーン容積(30-40 mL)に相当する血液量を心臓から吸引　→　駆出抵抗減弱　→　収縮期血圧が低下　→　心仕事量・心筋酸素消費量が低下

PCPS施行時にIABPを併用することが推奨される理由

• 1. 後負荷の軽減
• 2. 組織血流の拍動化
• 3. PCPS離脱時のバックアップ

適応

• 急性心筋梗塞、不安定狭心症、心原性ショックで、冠循環の改善と心臓のポンプ機能を補助したい場合

禁忌

• 大動脈弁閉鎖不全、大動脈解離、腎機能不全、胸部大動脈の人工血管置換術後(12ヶ月以内)(ICU.233)
• 大動脈弁閉鎖不全症、大動脈の高度な硬化性病変の合併
