無呼吸の定義
定義
- 一晩7時間の睡眠中に「無呼吸」が30回以上認められ、この無呼吸が、ノンレム睡眠時にも出現。
- 昼間時の眠気などの臨床症状があり、睡眠1時間当たりの「無呼吸」が5回以上である場合。(つまり1時間あたりのAHIが5以上)
疫学
- 男性(男女比は5:1)。中高年。肥満者。いびきをかく。
- 閉塞型が最も多い。
病型
閉塞型睡眠時無呼吸症候群 obstructive SAS, OSAS
- 睡眠に伴い、軟口蓋から下咽頭周辺の狭窄・閉塞(上気道の狭小化を伴う病態)が原因となり、無呼吸・低呼吸を繰り返すもの。
- 原因：鼻中隔弯曲、咽頭癌、扁桃腺肥大、アデノイド過形成、顎顔面形態の異常(小顎症など)
- 無呼吸・低呼吸の都度、覚醒反応から呼吸再開が起こるため、日中の眠気を訴えることが多い。ないこともある。
- OSASに伴う頻回の覚醒はまた交感神経活動の亢進を伴うため、一過性血圧上昇ばかりではなく、糖・脂質代謝に対する影響を介し、動脈硬化の促進などにかかわることが示唆されている。
- →高血圧、糖尿病、狭心症、心筋梗塞、脳梗塞などの有病率↑
