概念

• 甲状腺C細胞（傍濾胞細胞）由来のカルシトニン産生性の悪性腫瘍

疫学

• 甲状腺悪性腫瘍の5～10％？　甲状腺癌の1-2%(YN.D-41)
• 男女比はほぼ1：1　　男女比=1:2(YN.D-41)　
• 発症年齢は30歳代と50歳代がピーク

病型

• 家族性
• 散発性

病因

• RET遺伝子の点突然変異

• 常染色体優性遺伝疾患の多発性内分泌腫瘍症2A型，同2B型とおなじ

病理

• 腫瘍細胞の多様な像

• 類円形から多角形のさまざまな形態をとる腫瘍細胞が、シート状、胞巣状、島状あるいは管状、濾胞状、乳頭状の構造を呈する

• 間質のアミロイド沈着

病態

• 高カルシトニン血症：腫瘍細胞がカルシトニンを産生

進展様式

• リンパ行性が多く、血行性もある。

症状

• 甲状腺腫脹

検査

• 血液生化学

• カルシトニン：高値
• 腫瘍マーカー：CEA

• シンチグラム

• 131I-MIBGによるシンチ：腫瘍に集積

合併症

• 甲状腺髄様癌の1/3にMEN 2A(Sipple症候群)が、逆にMEN 2Aの60-95%に甲状腺髄様癌が存在する。

予後

• 10年生存率：60%
