Mole (モール) は、1989年にカメラ会社やフィルム会社が運営しない、写真専門自主ギャラリーとして開設された。

当初は「FROG（Film ROund Gallery）」という名前であったが、1991年よりこの名前になる。

四ッ谷三丁目の元雀荘だった建物に入っていた。

1990年代の東京の自主ギャラリーの中心的存在であった。

1998年、ギャラリーの入る建物の近くに、写真図書館をオープンする。

2001年6月、閉鎖された。

牛腸茂雄、春日昌昭など不世出の写真家を多く紹介した。

ギャラリーとしての活動以外にも写真集の販売、出版も行い、金村修の写真集を初めて出版した。

この写真集の販売部門は後にPLACE Mに併設された蒼穹舍に引き継がれた。

概念

• 妊娠時に、妊卵由来の絨毛が水腫様変化を起こしたもの。
• 肉眼的に絨毛の嚢胞化がみられる。嚢胞化とは腫大した絨毛の短径が2mmを超えるもの(参考1)。　　⇔　　2mm未満で絨毛間質の水腫化が認められるものは顕微鏡的奇胎とし、胞状奇胎とはしないとする(参考1)。

疫学

• 日本を含む東南アジア、メキシコに多い。
• 頻度は350分娩に1胞状奇胎である。
• 発生数は生殖年齢(20-30歳代)に多い。(G9M.182)
• 発生年齢は40歳以上の高年妊娠に多く発生し(10-30倍)、妊娠可能年齢の終わりに近づくほど高くなる。

分類

発生機序による分類

• 全胞状奇胎(complete hydatidiform mole)：無核卵子に1ないし2つの精子が受精して発生
• 部分胞状奇胎(partial hydatidiform mole)：健全な有核卵子に対して2つの精子が受精して発生

進展性による分類

• 非侵入奇胎
• 侵入奇胎
• 転移性奇胎

病因

• 雄性接合子：胎盤の形成に関与　→　倍加して二倍体の精子が染色体を欠いた卵子と受精した場合に胎盤のみが増殖　→　全胞状奇胎(complete hydatidiform mole)
• 雌性接合子：胎芽の形成に関与　→　高齢になるに従い、卵の加齢による染色体を欠いた異常な卵の出現頻度が高まり全胞状奇胎の原因となってくる。

症候

HBEsT (NGY.243)

• 1. history(病歴): 妊娠の成立を示す所見(無月経、つわりなど)
• 2. bleeding(子宮出血)：切迫流産様の異常出血
• 3. enlargement and softness(子宮の増大と軟化)：妊娠週数に比して過度に腫大して軟化
• 4. toxemia(妊娠高血圧症候群)：浮腫、高血圧、タンパク尿

• 不正性器出血(90%)、悪阻(30-40%) (G9M.182)
• 部分胞状奇胎の場合には典型的な症状を示さないことが多い。
• 黄体嚢胞による卵巣腫大が認められる(NGY.243)

検査

• 経腟超音波検査：子宮腔内の大小の低輝度・高輝度混在

治療

• 子宮内容除去術を施行し、1週間後に再施行する。hCGを5週(10³mIU/ml)、8週(10²mIU/ml)、20週(cut off値以下)となれば、経過順調型(I型)とされる。以降、4年以上はhCG測定、基礎体温測定、胸部X撮影を行い、フォローする。(NGY.243)
• hCG値の低下が悪い場合、子宮内胞状奇胎遺残、進入胞状奇胎、転移性胞状奇胎が考えられ、それぞれ子宮内容除去術、化学療法もしくは子宮摘出術、化学療法を行う。
• 40歳以上では絨毛癌の頻度が高くなるため、40歳以上で挙児希望の場合は子宮全摘術を考慮。(G9M.182)

• 手術療法：子宮内容除去術
• 化学療法：メトトレキサート、アクチノマイシンD、エトポシドによる単剤化学療法　⇔　絨毛癌の場合はEMA/CO療法

続発症

• 侵入奇胎：子宮内容除去術後、6ヶ月以内に発生することが多い。全胞状奇胎 十数%、部分胞状奇胎 約3%
• 絨毛癌　：全胞状奇胎の1-2%に発症。部分胞状奇胎では稀
