まとめ
- ベーチェット病の特殊病型の一つであり、消化管に病変を生じたものをいう。ベーチェット病に典型的な「口腔内アフタ、陰部潰瘍、虹彩毛様体炎」、結節性紅斑の他に腸管に病変を来す。症状は、腹痛、下痢、下血であり、全身症状として発熱も伴いうる。血液検査では炎症所見(CRP、WBC、ESRの上昇)が認められ、腹部X線造影では、典型的には回盲部に円形ないし類円形の打ち抜き像で下掘れ傾向が強く深い潰瘍が認められる。特異的治療無く、栄養療法、薬物療法(サラゾスルファピリジン、副腎ステロイド、免疫抑制薬、抗TNF-α抗体)が行われるが、難治である。手術療法は穿孔例、瘻孔形成例、出血例で適応となるが、半数は再手術が必要となる。(IMD.863 YN.A-72)
