Severe Community-acquired Pneumonia Caused by <i>Acinetobacter baumannii</i> Successfully Treated with the Initial Administration of Meropenem Based on the Sputum Gram Staining Findings: A Case Report

臨床経過で定義される疾患概念。
経過が急激であり、速やかに急性肺障害 ALI, 急性呼吸窮迫症候群 ARDSを引きおこして肺機能が低下し、呼吸管理が必要となる致命的な肺炎をいう。
リスク因子：免疫低下した患者(脾摘後、脾臓低形成、糖尿病、先天性免疫不全症候群・後天性免疫不全症候群)
疾患としては肺炎球菌性肺炎、レジオネラ肺炎がありうる。
レジオネラ・ニューモフィラによるレジオネラ肺炎は他の細菌性肺炎と異なり進行性が早いのが特徴である。初発症状は全身倦怠感、頭痛、食欲不振、筋肉痛。次いで乾性咳嗽(次第に痰を伴う)、高熱、悪寒、胸痛が出現する。中枢神経症状(傾眠、昏睡、幻覚、四肢の振戦振せんなど)が早期に出現するのも特徴。
肺炎に対する抗菌薬治療、その他肺障害を軽減する目的でステロイドを併用することがある。
