「解離性脳動脈瘤」

同: 脳動脈解離症脳動脈解離 cerebral arterial dissection、解離脳性動脈瘤

40-50歳代の男性に好発。

解離性動脈瘤に比較的多いのが、椎骨動脈閉塞による延髄外側症候群(ワレンベルグ症候群 Wallenberg syndrome)

病態

動脈壁が解離することにより壁内に血管が入り込む　→　動脈瘤様に拡張/血管腔の狭窄・閉塞

症状

頻度は1>2

1. クモ膜下出血で解離性脳動脈瘤を発症
2. 脳虚血症状で解離性脳動脈を発症　→　突然の頭痛・頚部痛に引き続き小脳、脳幹梗塞を来たす。
(その他)頭痛・頚部のみで解離性脳動脈を発症

椎骨動脈閉塞ではワレンベルグ症候群をきたす。

内頚動脈。(終枝？)

走行

大脳半球の側頭葉の溝の中を上方に枝を出しながら後方に向かう

分布

側頭葉：外側面の上方。内側面の前方
前頭葉、頭頂葉、後頭葉：島を中心に扇状に広がる
