大腸癌、便潜血反応検査、糞便潜血検査

目的

• 消化管出血のスクリーニング

• 免疫法を用いる場合には下部消化管の出血のスクリーニングに用いられる。

方法

• 化学法：ヘモグロビン(ヘマチン)が有するペルオキシダーゼ活性を利用して次の反応を触媒する：過酸化水素＋フェノール性物質→キノン型物質(青色呈色)。食事制限が必要(食物由来の血液に反応するため偽陽性が高い)で、全消化管の出血を検出するデメリットがある。

• オルトトリジン潜血反応試験 o-トリジン法
• テトラメチルベンチジン法
• グアヤク潜血反応試験
• ベンジジン法：発ガン性物質が含まれるので使われていない。

• 免疫法：ヒトヘモグロビン(抗原はグロビン部分)を特異的に検出するので、偽陽性が少なく、食事制限が不要というメリットがある(下部消化管出血に対して感度・特異度高い)。上部消化管の出血は検出しづらい。大腸集検に用いられ、2日法では進行癌の90%、早期癌の50%が陽性となる。

• 便潜血反応による大腸癌スクリーニング法(免疫法らしい)：進行大腸癌では、1回法による感度は70%、2回法による感度は80%。(出典不明)
