74歳男性．腹痛と血便のため当科を受診した．既往症に腹腔動脈上腸間膜動脈閉塞症を有していたが，下腸間膜動脈から上腸間膜動脈起始部への側副血行路を形成し，このルートがほぼすべての消化器系の腹腔内臓器への血液供給路となっていた．今回，下部内視鏡検査にてS状結腸癌を認め，腸閉塞を併発し緊急開腹術となった．まれな血行動態を把握した上で，安全にS状結腸切除術が施行できた1例を経験したので報告する．
