- 拡張終期に心房が収縮して血液が心室に流れ込む。(心室への血液の流出が進むと僧帽弁や三尖弁は)ふらふらと弁尖同士が寄り沿う(心室の血液のせい?)。心室が収縮すると弁尖同士はびたっと寄り添い閉鎖する。
- PR intervalが短いと、心室の収縮の前に弁尖同士はお互い離れており(受動的に寄り添っていないので)、心室収縮時の弁尖の移動距離は長い。このため大きいS1が聴取される。
- 僧帽弁狭窄症や三尖弁狭窄症では、心室が収縮するときでも心室と心房の圧較差は大きく、房室弁の弁尖は性状の場合に比べお互い離れているから、S1が大きくなる。
- 心拍数が高いとき、拡張期が短く、また弁尖がお互いよりそう時間が短い。このため、弁尖同士は離れておりS1が大きくなる
I音の減弱

- 僧帽弁閉鎖不全症ではお互いの弁尖が接触する面積が小さくなっており、I音の減弱に繋がっている。
- 重度の僧帽弁狭窄症では、弁がほぼ固定されてS1が減弱する
- 心室のコンプライアンスが低下する病態で、拡張末期の心室と心房の圧較差が大きくなるので通常より早く弁尖同士が寄り添う。心室が収縮する頃には健常人より弁尖同士が近くなっており、S1が減弱する。
巨大なI音
- 完全房室ブロックの時、心房と心室の収縮が同時に収縮すると大きなI音として聞こえる。

- 完全房室ブロックでは心房と心室が独立して収縮するので、心室への血液充満が刻々と変化するため、I音も変化する、のだろう。たまたま良いタイミング(P波の0.15-0.2秒後にQRSが出現するとき)で心室が収縮すると、大砲音を生じる。
