発症が乳児期？　→　乳児型
狭窄部は動脈管よりも近位。
動脈管は開存していることが多い
上半身：左心室からの血流。下半身：動脈管からの血流(合併する心奇形により、酸素化された肺静脈血が混合されている)。

→　(時に)下半身チアノーゼ

左室圧負荷　→

→　(1) 左右シャント増加　→　肺高血圧の出現。
→　(2) 心不全

管後型

発症が成人？　→　成人型
狭窄部は動脈管より遠位
動脈管は閉鎖？
上半身の高血圧、下半身の低血圧が特徴的で、側副路が発達し、年長児では肋間動脈が拡張(rib-notching)
左室圧負荷

治療

薬物療法

管前型の場合は直ちにプロスタグランジンE1を投与し、動脈管の閉鎖を防ぐ。

手術療法

いずれの方法でも動脈管は結紮、切離。

縮窄切除・単々吻合(resection with end-to-end anastomosis)：狭窄部を切除し、断端を吻合する。
鎖骨下動脈フラップによる大動脈形成術(subclavian flap aortoplasty)：鎖骨下動脈を切断し、その血管壁を利用して大動脈縮窄部位を拡張する。
Blalock-Park法：鎖骨下動脈を離断して狭窄部の遠位部に吻合。
人工血管置換術：成人に対して行われる。大動脈瘤/破裂の頻度が高い。
