概念

• 食道に発生する腫瘍。予後は不良な場合が多く、早期からリンパ節転移を来しやすい。

疫学

• 罹患率、死亡率共に男性の方が多い。
• 40歳後半より増加。60歳以上で多い(参考2)。増加の程度は男性の方が急増。
• 死亡率：漸減傾向
• 罹患率：(男性)増加傾向、(女性)変動なし。

危険因子

• 喫煙、飲酒、加齢

• 喫煙と飲酒とは相乗効果があるり、1日20本以上かつ3合以上でリスクが上昇する。
• 飲酒は特に度数の高い、アルコール摂取量が多い、ALDH2変異がある、顔面が紅潮する場合にリスクが上がる

• 熱い食事の嗜好、肥満、p53変異
• 食道アカラシア、腐食性食道狭窄、バレット食道
• 低栄養(動物性蛋白質の不足？微量元素欠乏？)、

病期分類

早期癌・表在癌・進行癌

• 早期癌：深達度が粘膜内に限局。リンパ節転移の有無を問わない。
• 表在癌：深逹度が粘膜下層までに限局。リンパ節転移の有無は問わない。
• 進行癌：固有筋層以降

日本

• 50-60%：胸部中部食道
• 20-25%：胸部下部食道
• 10%　 ：胸部上部食道
• 5-6%　：頚部食道

欧米で増加している食道腺癌の好発部位：下部食道

進展様式

• 直接浸潤
• リンパ行性転移：早期からリンパ節転移しやすい。

• 癌腫が粘膜下層にとどまるT1bで30-40%の例で転移。(SSUR.455)

m3・sm1ではリンパ節転移が10%の例で見られる。(出典不明)　　　⇔　m1・m2ではリンパ節転移を考えなくても良く、内視鏡的粘膜切除術(EMR)の絶対適応となる。

sm2・sm4ではリンパ節転移が約半数の例で見られる。(出典不明)

• その他

病理

• 日本では扁平上皮癌が中部食道に多い、欧米では腺癌が下部食道に多い。

肉眼分類の傾向

• 早期癌：0-IIc(表在陥凹型)
• 進行癌：2型(潰瘍限局型)・3型(潰瘍浸潤型)

症状

• 胸骨後方の軽い痛み、不快感、嚥下時のつかえ感、しみる感じ

検査

• 腫瘍マーカー：

(食道扁平上皮癌)SCC, CYFRA21-1 , CEA (参考3)

(食道腺癌)CEA, CA19-9 (参考3)

治療

内視鏡的切除

• 内視鏡的切除の適応：(ガイドライン1)

• 絶対適応：

• 壁深達度：T1a-EP(粘膜上皮),LPM(粘膜固有層))であること(リンパ節転移がまれなため)で、周在性2/3以下のもの(全周性に粘膜切除した場合、瘢痕狭窄のおそれ)

• 相対適応：(1) or (2)

• (1) T1a-MM(粘膜筋板), T1b-SM1(＜粘膜下組織200um)で、かつリンパ節転移がない
• (2) 周在性2/3以上の病変。　

術式

• 内視鏡的粘膜切除術 endoscopic mucosal resection, EMR
• 内視鏡的粘膜下剥離術 endoscopic submucosal disection, ESD

予後

• 手術後の在院死亡率3-4%。これは消化器癌手術の中で最も手術リスクが高い。(SSUR.464)
• 全切除全症例の5年生存率は36%(1988-1999年)。3領域リンパ節郭清例の5年生存率は約50%。(SSUR.464)
