概念
- 急激に発症する激しいぶどう膜網膜炎を特徴とする疾患(3)。急性汎ぶどう膜炎。
- 水痘帯状疱疹ウイルスあるいは単純ヘルペスウイルスの眼内局所感染によって発症する急性後部ぶどう膜炎で、周辺網膜の壊死を伴う。
- 1971年浦山晃らが桐沢型ぶどう膜炎として初めて報告(3)
病因
- 単純ヘルペスウイルス or 帯状疱疹ウイルス
疫学
- 健康な人に発症。
病変形成＆病理
病態
病理
- 硝子体混濁 ＋ 眼底の網膜に黄白色の滲出斑が出現、癒合 → 網膜の壊死 → (1-2ヶ月後)網膜の萎縮・変性 → 多数の網膜裂孔 → 網膜剥離 (3)
- 静脈に沿った帯状の出血(3)
症状
- (発症時)片眼の充血、眼痛、視力低下(3)
診断
- 眼底所見：眼底網膜に黄白色の滲出斑。静脈に沿った帯状の出血
- 前房水：ヘルペスウイルスDNAのからの検出
- 前房水 or 硝子体液：ヘルペスウイルス抗体価
検査
蛍光眼底造影
- 眼底周囲の網膜血管の血行が途絶(3)
治療
薬物療法
- 抗ウイルス薬(アシクロビル)
- ステロイドの全身投与
手術療法
- 硝子体手術(網膜の復位)
