Authorized and funded at $1.5 billion for 5 years, the MIECHV represents a large investment in health and development outcomes for at-risk children through evidence-based home visiting programs. The MIECHV presents unprecedented opportunities to integrate early childhood services systems, not only on the federal level but also within states and local communities. The MIECHV is funded in escalating amounts over 5-year period authorized, as follows: $100 million in fiscal year (FY) 2010, $250 million in FY 2011, $350 million in FY 2012, $400 million in FY 2013, and $400 million in FY 2014. Most of the funding is being provided to states and territories to provide home visiting services in their at-risk communities. In addition, the legislation included a 3% set-aside for tribes, tribal organizations, and urban Indian organizations and a 3% set-aside for research and evaluation. This investment has spurred the creation of more comprehensive and coordinated early childhood service systems across the United States. This article provides an overview of the MIECHV program, including descriptions of the various requirements under the Affordable Care Act. These include partnering with states to provide evidence-based home visiting services to at-risk families, working with tribal communities to implement culturally competent home visiting programs, and developing a mechanism to systematically review the evidence of effectiveness for home visiting program models and to conduct a national evaluation of the MIECHV program.

第14条
- 都道府県の区域を単位として設立された公益社団法人たる医師会の指定する医師(以下「指定医師」という。)は、次の各号の一に該当する者に対して、本人及び配偶者の同意を得て、人工妊娠中絶を行うことができる。 ← 母体保護法指定医
- 一 妊娠の継続又は分娩が身体的又は経済的理由により母体の健康を著しく害するおそれのあるもの ← 中絶手術の99.9%がこの号を適応事由としている。
- 二 暴行若しくは脅迫によつて又は抵抗若しくは拒絶することができない間に姦淫されて妊娠したもの
- 2 前項の同意は、配偶者が知れないとき若しくはその意思を表示することができないとき又は妊娠後に配偶者がなくなつたときには本人の同意だけで足りる。
- 第六章 届出、禁止その他
第25条
(届出)
- 医師又は指定医師は、第三条第一項又は第十四条第一項の規定によつて不妊手術又は人工妊娠中絶を行つた場合は、その月中の手術の結果を取りまとめて翌月十日までに、理由を記して、都道府県知事に届け出なければならない。
第26条
(通知)
- 不妊手術を受けた者は、婚姻しようとするときは、その相手方に対して、不妊手術を受けた旨を通知しなければならない。
第27条
(秘密の保持)
- 不妊手術又は人工妊娠中絶の施行の事務に従事した者は、職務上知り得た人の秘密を、漏らしてはならない。その職を退いた後においても同様とする。
第28条
(禁止)
- 何人も、この法律の規定による場合の外、故なく、生殖を不能にすることを目的として手術又はレントゲン照射を行つてはならない。
