概念
- 呼吸細気管支領域の慢性炎症性病変が肺内にびまん性に分布し、炎症反応が細気管支壁の全層に及ぶ原因不明の疾患。慢性の咳嗽・喀痰が認められ、慢性気道感染と拘束性の肺機能障害が認められる。副鼻腔炎の合併が認められることがあり、副鼻腔気管支症候群としてまとめられることがある。
- 胸部単純X線写真上、下肺野優位に両肺野びまん性小葉中心性粒状陰影が認められる(小葉中心性病変)。
- これまでは予後不良であったが、エリスロマイシン少量投与で著明な治療効果が認められる。

病態
病変の首座は終末細気管支及び終末細気管支周囲炎(出典不明)- 呼吸細気管支を中心とした細気管支およびその周囲炎 (YN.I-76)
- 泡沫細胞の集簇を伴ったリンパ球、形質細胞など円形細胞浸潤

検査
- 胸部単純X線写真：気管支拡張がみられる
- 胸部単純CT：下肺野優位に両肺野びまん性小葉中心性粒状陰影 ← 小葉中心性病変 (cf.小葉)
- 肺機能検査：一秒率低下、残気量増加、肺拡散能不変。
合併症
治療
- 予後は不良であったが、エリスロマイシンの少量長期投与により予後が改善 30年生存率38-63%
