Anatomical terminology |
The posterior ligament of the lateral malleolus (posterior tibiofibular ligament, posterior inferior ligament). It is smaller than the anterior ligament of the lateral malleolus and is disposed in a similar manner on the posterior surface of the syndesmosis.

A high index of suspicion is essential for diagnosis. High ankle sprains are managed symptomatically, with prolonged rehabilitation. The posterior inferior tibiofibular ligament is the strongest syndesmotic ligament; isolated injury of it is rare. We present 3 cases of isolated posterior high ankle sprain and discuss the relevant anatomy, mechanism of injury, and management.

Progressive collapse following open reduction and internal fixation (ORIF) has been described and can lead to failure in treatment. A standard anterolateral approach to the tibia may not allow direct reduction and stabilization of posterolateral joint depression given the anatomic barriers of the fibular collateral ligament and the proximal tibiofibular articulation (1). Posterolateral approaches to the tibial plateau have been described, and may allow direct reduction of the articular depression (2-7). These approaches, however, require dissection close to the common peroneal nerve and some approaches also require a proximal fibular osteotomy. The use of an intraosseous fibular shaft allograft as an adjunct to ORIF in select cases of depressed posterolateral tibial plateau fractures allows both reduction of the joint as well as stabilization of the articular segment through a single approach familiar to many orthopaedic surgeons.
