When they occur, they can interrupt normal articular congruency, leading to pain and joint dysfunction. If these conditions result in large osteochondral defects, they pose challenging reconstructive problems in young patients. We describe a case of a 29-year-old man who presented with a 2-year history of right hip pain. Advanced imaging demonstrated an expansile lesion in the region of his ligamentum teres (LT), eroding a significant portion of his femoral head and expanding the cotyloid fossa. He was treated with surgical hip dislocation, excision of the lesion, and femoral head reconstruction with fresh osteochondral (OC) allograft transplantation via press-fit technique. Histologic examination of the mass showed a benign fibromyxoid pseudotumor. Although non-neoplastic masses have been described in almost all organ systems, to our knowledge this is the first description of this entity affecting the native hip joint. It is only the second description of using press-fit OC allografting in the femoral head. This case adds to the body of literature defining symptomatic LT pathology that may benefit from surgical management. It underscores the need to study the ligament further, as the ability to diagnose and treat intra-articular hip pathology has improved with modern imaging and methods of open and arthroscopic hip surgery.

The aim of this study was to provide a clear anatomical interpretation of these structures. We dissected 40 fresh-frozen cadaveric knees to view the relevant anatomy and identified a consistent structure in 33 knees (83%); we termed this the anterolateral ligament of the knee. This structure passes antero-distally from an attachment proximal and posterior to the lateral femoral epicondyle to the margin of the lateral tibial plateau, approximately midway between Gerdy's tubercle and the head of the fibula. The ligament is superficial to the lateral (fibular) collateral ligament proximally, from which it is distinct, and separate from the capsule of the knee. In the eight knees in which it was measured, we observed that the ligament was isometric from 0° to 60° of flexion of the knee, then slackened when the knee flexed further to 90° and was lengthened by imposing tibial internal rotation. The valgus deformity is sustained by anatomical variations divided into bone remodelling and soft tissue contraction/elongation. Bone tissue variations consist of lateral cartilage erosion, lateral condylar hypoplasia and metaphyseal femur and tibial plateau remodelling. Soft tissue variations are represented by tightening of lateral structures: lateral collateral ligament, posterolateral capsule, popliteus tendon, hamstring tendons, the lateral head of the gastrocnemius and iliotibial band. Complete pre-operative planning and clinical examination are mandatory to manage bone deformities and soft tissue contractions/elongations and to decide if a higher constrained prosthesis is necessary. Two different approaches have been described to perform TKA in a valgus knee: the anteromedial approach and the anterolateral one. In valgus knee deformity bone cuts can be performed differently in order to correct low-grade deformities and reduce great deformities. There is still debate in the literature on the sequence of lateral soft tissue release to achieve the best alignment without any instability. The aim of this article is to review the anatomical variations underlying a valgus knee, to assess the best pre-operative planning and to evaluate how to choose the grade of constraint of the implant. We will also review the main approaches and surgical techniques both for bone cuts and soft tissue management. Finally, we will report on our experience and technique.
