症例は80歳代男性．心窩部の激痛を主訴に受診し，Amy高値とCT所見から急性膵炎と診断された．CT・MRIにて十二指腸下行部を取り巻くように腫大した膵頭部を認め，ERCPにて輪状膵およびいわゆる膵胆管高位合流と診断した．輪状膵には合併奇形が多いとされているが，輪状膵にいわゆる膵胆管高位合流もしくは膵胆管合流異常を合併した症例の報告は極めて少ない．両者の合併は膵の発生の観点からも興味深く，報告する．
