「心房粗動」
概念
- 心房拍数が240-440/分の規則正しい上室頻拍
- 心房で興奮が旋回することにより発生(リエントリー)。例えば三尖弁輪など。
疫学
- 60歳以上に多い
- 基礎心疾患や開心術の既往があることが多い。
心電図
- 着目する誘導：P波 = II誘導、F波：II, III, aVF
- 1. P波が存在しない = 基線が存在しない
- 2. 鋸歯状波、F波(250～350/分程度) ← 心房細動ではf波
- 3. RR間隔は整。房室伝導比=4:1,3:1,2:1 ← 心房細動では不規則
分類 (1)
- type 1：心房拍数 = 240-340/分の比較的に遅い粗動
- 多くが峡部依存性心房粗動(CTI-dependent atrial flutter)：下大静脈と三尖弁輪間の解剖学的峡部 cavotricuspid isthmus CTI を含む三尖弁輪を興奮が周回する右房内リエントリー
- type 2：心房拍数 = 340-440/分の速い粗動
症状
- 房室伝導比に依存する。(1)
- 1:1房室伝導 = 心拍数 約300/min：血圧低下、失神などの重篤な状態へ ← 運動時、房室電津尾が良好な場合
- 2:1房室伝導 = 心拍数 約150/min：動機、呼吸困難、胸痛、心不全、血圧低下
- 4:1房室伝導 = 心拍数 約 75/min：無症候
合併症
- 血栓塞栓症。頻度は心房細動の1/3 (1)
治療
- 血行動態安定 ：洞調律への復帰(DCショック、心房ペーシング、抗不整脈薬) ± レートコントロール(HR≧100/minの時、房室結節抑制薬：ジゴキシン、β遮断薬、Ca Ch blocker(ベラパミル、ジルチアゼム))
- 血行動態不安定：DCショックで洞調律に復帰させる。
