疫学

• 日本では肝膿瘍の90%以上が細菌性肝膿瘍

感染経路

• 特発性
• 浸潤性・外傷性
• 血行性(経門脈性)
• 胆道性：結石、胆管炎、胆汁うっ滞
• 医原性：肝動脈塞栓術後、局所焼灼治療後

特徴

• 経胆道性：多発性
• 経門脈性や外傷性、特発性：単発性(右葉に好発)
• ウイルス性肝膿瘍に比べて多発性が多い

病原体

• グラム陰性桿菌(Escherichia　coli)が最多

症状

• 弛張熱、右悸肋部痛、脱力感、嘔吐

検査

• 血算：白血球増多
• 血液生化学：CRP上昇
• 肝機能：障害は軽微
• 腹部超音波検査：低エコーの腫瘤
• 腹部単純CT：低吸収の腫瘤

治療

• 抗菌薬
• 原因除去：胆道ドレナージなど
