定義
- It refers to a descrease of systolic bood pressure(more than 10 mm Hg) during normal inspiration
病因
病態生理
- 正常な人手も吸気時に収縮期血圧が低下するが、いくらなんでも下がりすぎ・・・というのが奇脈。
- 吸気→↑静脈還流量→右室拡張→心室中隔を左方圧迫→↓左室駆出量→↓血圧
- 心タンポナーデでは、心室の拡張が妨げられるが、こういう状況の時、吸気時に右室が心室中隔を左室に向かって押す作用が(呼気時にと比較して)相対的に大きくなるため (PHD.343)
- 収縮性心膜炎の場合は、奇脈は限定的
