概念

- アンジオテンシンIIの受容体はAT1とAT2がある。アンジオテンシンIIの生理作用としては血管収縮、アルドステロンの分泌、腎臓におけるNa+再吸収促進、交感神経系の刺激などがある。これらの作用はAT1受容体を介している(AT2受容体は胎児期の発生の際には豊富にあり、成人の一部臓器にも分布しているが、生理機能は不明)。アンジオテンシンII受容体拮抗薬はこのAT1受容体に競合的に結合して降圧をはかる薬剤である。血液中のアンジオテンシンIは循環血液中のACE以外の酵素によってアンジオテンシンIIに転換されうるので、アンジオテンシンII受容体拮抗薬の方がアンジオテンシン転換酵素阻害薬よりRAA系をより阻害するとされている。

- アンジオテンシンⅡ受容体拮抗剤並びにアンジオテンシン変換酵素阻害剤で、妊娠中期～末期に投与された患者に胎児死亡、羊水過少症、胎児・新生児の低血圧、腎不全、高カリウム血症、頭蓋の形成不全、羊水過少症によると推測される四肢の拘縮、脳、頭蓋顔面の奇形、肺の発育形成不全等があらわれた。
