- Multidisciplinary management in Merkel cell carcinoma.
- Matkowski R, Lata E, Zietek M, Halon A, Forgacz J, Szynglarewicz B.Author information Department of Oncology, Division of Oncological Surgery, Wroclaw Medical University , Plac Hirszfelda 12, Wroclaw , Poland.AbstractHerein, the authors describe the experience with the treatment of Merkel cell carcinoma (MCC) and review the literature regarding MCC treatment regimens. Nine patients underwent treatment due to stage I, II, or III MCC. The median follow up was 39 months. In five cases, tumors were excised with skin margins of >2 cm, and skin margins were <2 cm in four patients. Local adjuvant radiotherapy (RT) was given to four patients, while three patients underwent local lymphadenectomy (LAD). Local recurrence occurred in four patients who did not undergo RT (among them three patients had excision margins <2 cm) after a mean time of 9 months. Despite retreatment, two of those patients developed metastases. Recurrence-free survival after primary therapy was achieved: (a) in three patients with stage I and II MCC treated surgically with excision margins <2 cm combined with RT or wide excision >2 cm alone and (b) in two patients with stage III MCC treated with wide excision and LAD combined with local and regional RT. A review of the literature supports the following recommendations: (a) excision with adequate margins combined with RT; (b) LAD with regional RT in cases of lymph node involvement; and (c) sentinel lymph node biopsy in patients without clinically suspicious lymph nodes.

- Repair of the donor areas defects after split-thickness skin grafts utilizing an advanced epithelialization dressing.
- Fanti PA, Dika E, Vaccari S, Misciali C, Ismaili A, Barisani A, Patrizi A.Author information Department of Internal Medicine, Division of Dermatology, Geriatric Diseases and Nephrology, University of Bologna , Bologna , Italy.AbstractIntroduction: The management of surgical wounds of the donor areas for split-thickness skin grafts might be difficult in terms of pain, risk of infection, and delayed healing. Materials and methods: In the period from September 2011 to June 2012, 12 patients affected by large ulcerated basal cell carcinoma (BCC) of the lower legs, 2 patients affected by squamous cell carcinoma (SCC), and 1 patient presenting malignant melanoma located on the scalp, with split-thickness skin grafts were treated. In the 12 BCC patients, the donor site areas were situated on the anterior or lateral site of the contralateral thigh and in 3 other patients on the forearms. Immediately after dermatome skin removal of 0.3 mm layer of epidermis, the donor sites were treated by applying an advanced epithelialization dressing consisting of pure cellulose film in all patients. The wound surfaces were covered by the cellulose dressing with 1 cm of excess. Bleeding was controlled with slight compression with sterile gauzes before the medication was applied. Multiple small cuts of the cellulose dressing, with a No. 11 surgical blade were made, in order to avoid hematoma formation or fluid accumulation. A mild compression bandage was used in all patients. The patients were visited after 7, 14, and 21 days. The bandage was removed after 1 week and the medical device was left at the site for other 7 days. Results: After 2 weeks, the medical device was partially detached on the underlying areas showing almost completely reepithelialization. Almost no pain was referred by the patients. In the authors' experience, this medical device markedly reduces the level of pain especially at the time of dressing application, after the surgical procedure, and during the healing process. No infections were observed. The healing process was fast and produced good aesthetic results. Conclusions: The use of Cuticell® Epigraft BSN medical device simplifies the management and healing of the donor areas for split-thickness skin grafts.

- Reconstruction of a large nasal defect.
- Goulão J.Author information Dermatology Department, Garcia de Orta Hospital , Almada , Portugal.AbstractA 77-year-old man was referred for treatment of a biopsy-proven multirecurrence basal cell carcinoma on the dorsum of the nose. The lesion was removed and the resulting surgical defect involved the full thickness of the nasal skin leaving intact the underlying perichondrium and cartilages of the whole nasal dorsum including both sidewalls (Figure 1). The authors performed a reconstruction of a surgical defect greater than two-thirds of the nose using a single-staged bilateral Burow's wedge flap and a full-thickness skin graft from the glabella. All scars were camouflaged in the natural facial boundaries and the surgical reconstruction was performed in a single stage. The final nasal contour, symmetry and function were still good when the patient was seen 1 year after surgery.
