- 全身性の血液凝固異常や抗凝固剤の服用、過排卵刺激に起因する局所の循環動態の破綻など
- 特発性卵巣出血
- 卵胞出血：排卵に伴う断裂血管からの出血
- 出血性黄体嚢胞：卵胞出血による血液が黄体内に貯留して血腫を形成し嚢胞化したもの
症状
疫学
- 12-52歳
症状
- 急性の下腹部痛
徴候
- 消化器：悪心、嘔吐、下痢
- 腹部：圧痛、筋性防御
診断
- 年齢、突然発症した腹腔内出血(超音波検査で確認)、妊娠反応陽性、予定月経の1週間前
- 排卵期(卵胞出血の場合)や黄体期(月経第15-28日)のタイミングで発症することが多い。
治療
- 全身状態が安定していれば、入院・外来で経過観察
- 大量出血や出血の持続がある場合、緊急手術 → 血算で確認
- 手術を要するのは全体の18%程度
