労作時・安静時問わず狭心痛が認められ、運動負荷試験では有意なST変化が生じているにもかからわず、冠動脈造影検査で有意狭窄を認めず、アセチルコリン負荷試験でも陰性である狭心症である。
閉経期前後(40-50歳)の女性で冠微小血管狭心症が起こり、これは女性に特有らしい。(参考1)
発作の誘因は、過労、ストレス、睡眠不足らしい。(参考1)
原因としては冠動脈分枝の微小血管の攣縮である(冠微小血管攣縮)。一方で、冠血管の微小血管に器質的な変化が見られ、これが発症に繋がる、らしい。
治療は硝酸薬が、予防にはカルシウム拮抗薬が試されている。β阻害薬も冠血管拡張薬と組み合わせれば使っても良い、かもしれない。有効な治療法は確立していないらしい。
