- Warm pediatric cardiac surgery: European experience.

However, the beneficial effects of cold perfusion are counterbalanced by the drawbacks of hypothermia. Thus, we propose a major paradigm shift from hypothermic surgery to warm perfusion and intermittent warm blood cardioplegia. This approach gives satisfactory results even with prolonged aortic crossclamp times. The major advantages are reduced time to extubation and shorter intensive care unit stay. Warm pediatric surgery is an anecdotal phenomenon no more; over 10,000 procedures have been carried out in Europe. All types of cardiopathy have been treated, including arterial switch, total pulmonary anomalous venous return, interruption of the aortic arch, and hypoplastic left heart syndrome. Once surgeons decide to shift from hypothermia to normothermia, they never decide to shift back to hypothermia. This fact is evidence of the satisfactory clinical outcome obtained with this technique. The technique and the composition of microplegia is identical in all European centers, the only variable factor being the interval between microplegia injections, which varies from 10 to 25 min. We hope that the increasing interest in pediatric warm surgery will hearten new candidates.

- Nissim Momigliano L.AbstractThe clinical case of a female patient who seemed to be demanding from the analyst changes in her usual technique but in reality wanted her to modify her emotional attitude induced the author to confront the subject of anxiety about change felt to be 'catastrophic', considering it on this occasion from the point of view of the analyst. It is inevitable that the analyst too will initially resort to his or her old defences, succeeding only later by a great deal of hard work in transforming them and putting them in the service of the analysis itself. The main subject of the paper is accompanied by a brief historical digression on the profound changes that have taken place in the years since Freud's death in the way the psychoanalytical movement as a whole thinks of and practises psychoanalysis, culminating in the 'pluralism' mentioned by Wallerstein, and by a personal example of the path travelled day after day by every analyst in his or her consulting room; the overall result is to hearten and encourage us about the future development of our work.

We have, among 90 families seen for genetic counseling between 1972 and 1985, estimated the risk of recurrence; it was of 0.25 in 55 cases; between 0.05 and 0.16 in 30 cases, greater than 0.01 in 16 cases. Before suggesting a prenatal diagnosis, it is necessary to take as basis the risk of abortion and the reliability of the method. The first risk is well known, less 0.005, in these young women; reliability can be estimated: almost 0.01 of false wrong negative, and this number is probably overvalued if all technical conditions are perfect. In front of these risks, it seems possible to propose a prenatal diagnosis from a risk of 0.01, the families being informed of all risks. We think so to hearten some families and to make possible for them to live quietly these pregnancies, probably non undertaken without our help.
