After tumor excision, the function of the eyelid has to be preserved, keeping the protection of the eyeball with preservation of damp, avoiding epiphora and ectropion. The authors describe the reconstruction, in a single surgical procedure, of a full-thickness defect of the outer half of the free edge of the lower eyelid through free chondromucosal graft of the septum and Tripier flap. There are very few cases that describe the septal chondromucosal flap in eyelid reconstruction. This case also corroborates the fact that the chondromucosal graft is a valuable surgical option for reconstruction of full-thickness lower eyelid defects.

The lesion was removed and the resulting surgical defect involved the full thickness of the nasal skin leaving intact the underlying perichondrium and cartilages of the whole nasal dorsum including both sidewalls (Figure 1). The authors performed a reconstruction of a surgical defect greater than two-thirds of the nose using a single-staged bilateral Burow's wedge flap and a full-thickness skin graft from the glabella. All scars were camouflaged in the natural facial boundaries and the surgical reconstruction was performed in a single stage. The final nasal contour, symmetry and function were still good when the patient was seen 1 year after surgery.

Patients and results: The authors present the case of an 89-year-old woman who underwent radical excision of basal cell carcinoma localized in the left scapha. The reconstruction of the resulting defect was performed using a preauricular transposition flap tunnelized through a cartilaginous fistula created at the crus helicis. The flap was deepithelialized at its base to allow the closure in a single step. The donor site was primarily closed. The procedure was performed under local anesthesia without complications and with acceptable aesthetic results. Discussion: There are various surgical procedures described for the reconstruction of the anterior auricle, including local flaps, skin grafts and even healing by secondary intention. The authors consider the tunnelized preauricular transposition flap a good option especially when compared to interpolated flaps, since it allows a reconstruction in a single surgical step with decreased morbidity and favorable cosmetic results.
