- Wan KS, Chang YS.Author information Department of Pediatrics, Taipei City Hospital , Renai Branch , Taiwan and.AbstractAbstract Background: The efficacy of the combination of leukotriene receptor antagonist (LRA) and H1 antihistamine was similar to the synergistic regimen of H1 and H2 antihistamine for treatment of chronic idiopathic urticaria (CIU). However, the effective rates of these two regimens were only 53.3% and 63.3%, respectively. Method: A total of 50 with two combined therapeutic regimens treatment ineffective patients were evaluated. Patients were single blinded and randomly assigned to one of two medication groups that received the following regimens for 4 weeks: Group A (n = 30), combination of LRA, H1 antihistamine and H2 antihistamine. Group B (n = 20) continued with the previously taken two combination regimens. The treatment efficacy was measured by daily urticaria activity score (UAS) of wheal and itch. A positive therapeutic response was defined as a reduction to <25% of baseline weekly UAS, while a relapse was a return to >75% of baseline weekly UAS. Results: At the end of 4 weeks, the UAS response to treatment of Group A was decreased from 35.2% to 21.2%, and the Group B was persisted with 33.9% as before the treatment. Conclusion: The combination of LRA, H2 antihistamine and H1 antihistamine is promising for the refractory CIU cases, which were refractory for two combined therapeutic regimens.

In general, Clinoleic droplets were smaller than Intralipid droplets, being around 255 and 285 nm, respectively, for blank and freshly loaded emulsions. Regardless of storage temperature, the Clinoleic exhibited a very slight or no increase in droplet size upon storage, whilst the droplet size of the Intralipid emulsion increased significantly. The droplet size of both emulsions was minimally affected by paclitaxel concentration within the range of 0, 1, 3 and 6 mg/ml. The pH of both emulsions markedly decreased upon storage at room temperature, which was possibly attributed to the production of fatty acids resulting from phospholipid hydrolysis. However, at 4 °C, the pH of Clinoleic emulsion was unaffected by storage or paclitaxel concentration while the Intralipid emulsion demonstrated a trend for pH reduction. Both nanoemulsions had a negative zeta potential, with the Clinoleic formulations having the highest charge, possibly explaining the better size stability of this emulsion. Overall, this study has shown that paclitaxel was successfully loaded into clinically licensed parenteral emulsions and that Clinoleic showed greater stability than the Intralipid.

Three formulations of PEG nanoemulsion, nanosuspension and an oil suspension containing puerarin were examined with the lymph-cannulated anaesthetized rat model. Plasma and lymph samples were analyzed by HPLC. Lymph triglyceride was measured using an enzymatic colorimetric technique. After 8 h, the total administered dose accumulated in the thoracic lymph duct was analyze. Nanoemulsion, nanosuspension and oil suspension was 0.065 ± 0.006%, 0.137 ± 0.018%, 0.021 ± 0.002% of the administered dose, respectively. In nanoemulsion, nanosuspension and oil suspension group, the systemic bioavailability of oral puerarin was 11%, 16% and 11% for lymph-cannulated rats, 41%, 67% and 18% for control rats. Absorption into the intestinal lymph should thus contribute to ∼30%, 51% and 7% of the systemic bioavailable puerarin. This data indicated that lipid-based nano drug formulation produced higher lymph concentrations of puerarin than oil suspension. The nanosuspension formulation may be considerable in terms of increased local concentrations in lymphoid tissue.

A sensitive, simple, and fast method for determining pyridaben in cabbage was established by high-performance liquid chromatography tandem mass spectrometry. The average recoveries were in the range of 90.29-95.00% with relative standard deviations ranging from 1.72% to 6.39%. The field results showed that pyridaben dissipated rapidly in cabbage and had a half-life of 2.8-3.5 d. During harvest, the terminal residues of pyridaben were 0.01-0.80 mg/kg. Given that no maximum residue limit (MRL) has been set for pyridaben in cabbage, risk assessment was evaluated by using the risk quotient (RQ). Results indicated that the RQ value was significantly lower than RQ = 1. Thus, the effect of pyridaben in cabbage at the recommended dosage was negligible to humans. This study could provide guidance for the safe and reasonable use of pyridaben as a broad-spectrum acaricide and serve as a reference for the establishment of an MRL in China.
