症例は62歳男性．右腎盂結石に対して体外衝撃波砕石術を受けた．施行中からの右季肋部痛と同日夜，膵酵素の上昇，CT上膵頭部の腫大認め，急性膵炎と診断された．翌日より尿量低下，胸腹水貯留を認め当院に転院となった．重症急性膵炎と診断し動注療法と持続血液濾過透析を含めた集学的治療により軽快し第30病日に退院となった．尿路結石に対するESWLの早期合併症の1つとして急性膵炎を考慮すべきである．
