Electronic address: domenico.corradi@unipr.it.AbstractAtrial fibrillation (AF), the most common sustained cardiac arrhythmia encountered in clinical practice, is associated with increased morbidity and mortality. Electrophysiologically, it is characterized by a high rate of asynchronous atrial cell depolarization causing a loss of atrial contractile function and irregular ventricular rates. For a long time, AF was considered as a pure functional disorder without any structural background. Only in recent years, have new mapping and imaging techniques identified atrial locations, which are very often involved in the initiation and maintenance of this supraventricular arrhythmia (i.e. the distal portion of the pulmonary veins and the surrounding atrial myocardium). Morphological analysis of these myocardial sites has demonstrated significant structural remodeling as well as paved the way for further knowledge of AF natural history, pathogenesis, and treatment. This architectural myocardial disarrangement is induced by the arrhythmia itself and the very frequently associated cardiovascular disorders. At the same time, the structural remodeling is also capable of sustaining AF, thereby creating a sort of pathogenetic vicious circle. This review focuses on current understanding about the structural and genetic bases of AF with reference to their classification, pathogenesis, and clinical implications.

- Translation and Cultural Adaptation of Patient Perception of Arrhythmia Questionnaire in Poland.

There is no valid translation of PPAQ available in Poland, which hinders research in this area with Polish arrhythmia patients. The aim of this study was to conduct initial content validity testing through translation and cultural adaptation of the English language version of the PPAQ to the Polish language. Methods: The whole project was conducted according to ISPOR Principles of Good Practice for the Translation and Cultural Adaptation Process for Patient-Reported Outcomes (PRO) Measures published in 2005. Results: In 2011, the PPAQ was translated into Polish and cultural adaptation was performed on 20 patients with SVT (12 male, age 54.9±17.4). Issues concerning close meanings of symptom names and language-dependant gender-related distinctions were identified. The former was solved by cooperation with experts in arrhythmia and latter by incorporating patients' preferences during cognitive debriefing. Conclusions: The Polish translation was well accepted by patients during this translation and initial content validity testing. Issues arisen during the translation process may recur in other translations and be resolved in similar manner.

Here we report a novel mutation of VHL in a patient diagnosed with malignant pheochromocytoma at the age of 17.Patient and Methods: A 17-year-old female was referred for paroxysmal supraventricular tachycardia and anemia. She was diagnosed with a left adrenal pheochromocytoma based on biochemical and imaging studies. A left adrenalectomy was performed. Six months after surgery, metastatic lesions were suspected in the lung. The histopathologic findings of thoracoscopic lung biopsy specimens confirmed metastasis of the malignant pheochromocytoma.Results: Genetic analysis of VHL showed a novel missense mutation at codon 194 (V194G) in exon 3. This mutation was inherited from the paternal allele, and showed a loss of heterozygosity in three distinct tumors of the patient. Two independent in silico analyses suggested that this amino acid substitution was pathogenic.Conclusion: We identified a novel missense mutation of VHL in a young patient with malignant pheochromocytoma.
