動揺関節、糖尿病性神経障害性関節症

概念

• 感覚神経障害により引き起こされる関節症であり、関節軟骨の変性、骨破壊、および不規則な反応性の骨増殖をきたす。(SOR.240)

原因

• 脊髄癆、糖尿病(足部関節に多い)、脊髄空洞症、脊髄髄膜瘤、癒着性くも膜炎、脊髄損傷、多発性末梢神経炎、先天性痛覚異常症

病態

• 痛覚、深部感覚などの関節の体性感覚障害により、関節は生理機能を障害されて過度の負荷や外傷を受けやすくなる。この結果として関節軟骨の変性、骨破壊、および不規則な反応性の骨増殖をきたす。(SOR.240)

好発部位

• 膝

症候・身体所見

• 感覚神経障害、関節の腫脹、疼痛、不安定感

検査

• 単純X線写真：骨破壊、関節面の不規則な硬化像、骨形成像

治療

• 装具固定、関節固定
• 人工膝関節全置換術は人工関節部分の折損やゆるみが生じ易く、適応の適否については議論中(SOR.589)
• 進行例では外科手術は避けた方がよい。奏効しないから。(uptodate.1)
