概念

• 出産直後から進行性の呼吸不全を起こす代表的疾患。
• 胎児の未熟性(在胎34未満)による肺サーファクタントの産生障害により、肺胞の虚脱および硝子膜形成に基づく換気障害である。肺胞の拡張不全によりアシドーシスを来たし、肺血管の収縮により低酸素血症が悪化する。また、アシドーシス・肺血流減少、肺サーファクタント不足により肺胞上皮が障害をうけ、透過性の亢進により肺硝子膜が形成される。結果として呼吸不全となる。(SPE.99 NGY.503)

疫学

• 未熟児・早産児にみられる(34週以下に多い)
• 未熟児、母体糖尿病児、帝王切開児に多い。 (QB.O-84)

症状

1-4が呼吸窮迫症候群の四徴とされる

• 1. 陥没呼吸
• 2. 呻吟
• 3. 多呼吸
• 4. チアノーゼ
• 2. 尾翼呼吸

検査

胸部単純X線写真

SPE.101

Bomsel分類:I～IV度で判定。IV度は真っ白で無気肺が認められる

• 肺胞の拡張不全
• 網状顆粒状陰影：空気の入っていない肺胞が黒く見える
• 気管支透亮像
• すりガラス状陰影

呼吸窮迫症候群	肺胞の拡張不全、網状顆粒状陰影、気管支透亮像、すりガラス状陰影
一過性多呼吸	肺野全体の透亮像低下(呼吸窮迫症候群より軽度)、肺尖部の高透過性
胎便吸引症候群	胎便による全肺野の炎症像(斑状、まだら状の不透亮像)

肺サーファクタント

• マイクロバブルテスト：肺から検体をとり、6sec混ぜて4sec静置し検鏡する。1mm2あたりの泡の個数を計測。
• L/S比の測定

診断

• 臨床症状
• 胸部X線

治療

• 軽度：酸素投与、人工換気療法、サーファクタント気管内注入、
• 中度：持続陽圧呼吸（CPAP）←呼吸終末にも陽圧をかけ肺胞虚脱を防止する。
