先天性肺動静脈瘻

• 病因：

• 先天性

• 特発性？
• 遺伝性：(症例の15%に)オスラー・ウェーバー・ランデュ病(=遺伝性出血性毛細血管拡張)が存在

• 後天性：肝硬変、肺住血吸虫、甲状腺癌肺転移、外傷

• 病態

• 肺動脈と肺静脈のシャント率が20%を超えると酸素化されない静脈血が体循環に入って諸症状(労作時呼吸困難、チアノーゼ、多血症、バチ指)を呈するようになる。
• 下肺野に多い

• 症状：

• 発症は小児
• チアノーゼ、ばち指、息切れ、喀血、胸痛、(30-50%の症例)皮膚粘膜の血管腫

• 増悪因子：

• 妊娠　　←　　女性ホルモンに血管増殖作用があるから？

• 合併症：

• 右左シャント：低酸素血圧症、二次性多血症　→　脳梗塞
• 動静脈奇形：肺動静脈奇形、脳動静脈奇形　→　脳出血
• 奇異性：奇異性脳膿瘍、奇異性脳塞栓

• 検査：

• 単純X線：円形状陰影
• 造影CT：拡張した流入血管とこれに連続する流出血管が描出される。
• 肺動脈造影：確定診断

• 治療：全例治療対象。切除、塞栓術
